NYS License – Electronic Verification of Experience

    Hi,

    Has anyone used electronic verification of experience and have their supervisor e-mail the form before? My office has been working from home since March so I have no way to reach my supervisor. I plan to ask my boss to do electronic verification. Anyone know if they will accept electronic signature also? Or have to be a scan of the form with a hand-written signature?

    Electronic was not an option in 2018.
    There was a form to fill out, sign and mail to an address.
    Any link to the electronic option?

    If it were me, I would ask the boss to sign it with blue ink, and then scan it in color.
    Just so there is absolutely no question that it was an original signature.

    They might accept an electronic signature, but why risk it?
    If your boss or supervisor doesn't have access to a scanner, then I'd risk the electronic vs nothing.
    No reason to cause any extra delay.

    This form must be submitted directly by the supervisor. This form will not be accepted if submitted by the applicant.

    Section I: Complete this section and forward the form along with a copy of Form 4BInst to your supervisor and ask that they complete Section II.
    Found it-
    Seems pretty straight forward, supervisor just has to complete and email the form.


    Section II: Have your supervisor complete this section, sign and date the affirmation and send the form along with any required documentation directly to the Office of the Professions at the address at the end of the form.

    Electronic Verification of Experience

    The Office of the Professions (OP) will accept experience forms directly from supervisors provided that:

    OP can independently verify that the documentation is received directly from the supervisor
    The applicant had no opportunity to directly alter the experience form before it is sent or transmitted.
    Any experience documentation submissions should be made electronically to DPLSExperience@nysed.gov*.

    Our office ban us from using our own printer / scanner when working from home….I search around and there's no where to call the state and ask, and the contact me e-mail function doesn't have this question category. I think this function is new due to COVID-19.

    I plan to ask my boss in coming weeks what the option is (and beg for the signature). My last resort is the very traditional mailing / sending a messenger method.

    Jfkgy can your supervisor submit the experience for via the email address, dplsexperience@nysed.gov, provided with the instructions (from the link you provided above) for him/her doing so? If not, why?

    Thanks all for your help.

    I was actually able to get to a representative in Albany and ask the question. They do take electronic signature! They also said they prefer e-mail certification because it's easier to track paperwork. The office is partially open but the representative said they are working remotely and haven't been in the office since March…
