Anyone apply for medical ADA accommodations?

    I have a diagnosed medical condition which might require accommodations during the exam depending on if I am having symptoms. I want to make sure my Dr writes the letter correctly so that this process goes quickly but I am a little confused by the wording in the form from nasba. If anyone else has been through this can you explain what type of info your Dr included in the letter? I pasted the letter requirements below that are confusing to me. When it mentions supporting documentations do they want the actual blood test results and then an explanation why the test results prove the diagnosis?? Or would a worded description by the Dr be sufficient?

    * Documentation must be submitted on official letterhead from a licensed or qualified professional who has examined the applicant and diagnosed a physical or mental impairment. Depending on the nature of the disability and written evaluation, documentation may include a letter from a physician or a detailed report.

    * Disability documentation must be detailed and specific. Documentation for all disabilities should describe the extent of the disability, the criteria for the diagnosis, the diagnosis, the type and length of treatment and the recommended accommodation. Terms such as “problems,” “deficiencies,” “weaknesses,” “differences” and “learning disability” are not the equivalent of a diagnosed specific disability (such as ADHD, Dyslexia, Multiple Sclerosis, etc.).

    * Include a recommendation for the specific accommodation with current and detailed documentation supporting the request

    * Provide evidence that similar accommodations have been made for the applicant in other educational or testing situations or in employment settings, or describe why no such accommodation was made in the past but is now required

    * The request for accommodations and appropriate supporting documentation should provide evidence of a substantial limitation to physical or mental (academic) functioning.

    Send an email to NASBA at cpaexam@nasba.org and ask what type of supporting documentation would be better suited for submission, even send copies of what you would like to submit for approval is you want. The ADA Coordinator would let you know what to submit.

    I believe you need to be more specific on what kind of medical condition you have and why you need accomodations. Is it physical or mental? Simply put, you cannot wing this with a little one paragraph doc's note with give me extra this or extra that. It's going to be 100% rejected. There has to be clear instructions by the doctor such as 1.25x extra time or 1.5x extra time and why extra time is needed and an explanation of why or how it is suppose to help the test taker. The report needs to be as detailed as possible, like literally minimum 10 pages. preferably on a letterhead with test results and analysis as well as an official diagnosis. Most of the reports are usually done by what's called a neuropsychological evaluation, which takes 3 full 8 hour days. It is also very expensive without insurance, at least $3000-5000 minimum. For those who just want to get extra accomodations that they can live without to get an advantage, you probably should not try at all. It is long process trust me on this one. There are pretty strict guidelines. GL! I can only help you further if I know what kind of condition you have as everyone is different.
