How do you know if the 2nd testlet is medium or difficult? And are you better off getting the difficult testlet as your 2nd one? How does it impact your chances of passing?

Sometimes you can tell, some times you can't.
I usually based second testlet time as compared to the first testlet time.
If the second took longer, I assumed more detailed or complicated calculations etc.

In theory, a harder 2nd testlet means more points you can achieve based on harder questions are worth more theory.
But, harder questions also take longer so… you eat more time leaving less time for SIMs.

My recommendation is to do it all as fast as possible, leaving as much time for SIMs as you can, specifically for FAR.
