I have a question regarding my application after passing all parts of the exam for California.
From my research it looks like I need to submit:

https://www.dca.ca.gov/cba/applicants/livescan.pdf4) Criminal Conviction Disclosure
https://www.dca.ca.gov/cba/applicants/disclose.pdf5) CALIFORNIA PROFESSIONAL ETHICS (PETH) EXAMINATION (Not yet completed)
I currently need to take 3 more accounting subject classes (online) which I am aiming to complete by the end of this year.
A) So do I just mail all forms + $250 check in one letter?
b) I current am an accountant a publicly traded company. Which experience form (public or nonpublic) do I fill out?Thank you
