Where is the experience sign off form for NY?

    Hi, I passed all the exam today!!! However, I am filling out my NY CPA application form. I notice there is nothing mention about the experience sign off form. Where is the form? Do I send the application in first then they give me the form? In the application, it does not mention about the ethic exam also. However, I purchased the Ethic exam online version just now. I am planning to finish the exam tonight and send in the application tomorrow. Is it how the process goes? Thanks.

    The experience form is Form 4B on this page: https://www.op.nysed.gov/prof/cpa/cpaforms.htm

    (Make sure you have all of the other applicable documents on that page as well.)

    Make sure whoever is certifying for you sends it in directly– they should not give it to you to send in.

    Are you sure you are supposed to take an ethics exam? It looks to me like they build that into CPE.

    Thanks for telling me that!!! I called the AICPA and is able to get a refund (Although it takes up to 3 weeks). I save $156.25. Thanks a lot!!
