NYS License Question–Submitted Form 4B with Form 1 :(

    Hi everyone! I was searching for a similar problem to mine but haven’t seen any so here goes:

    I finally completed all my exams and requirements. I filled out Form 1 and my employer filled out Form 4B and gave it back to me. I don’t know how I made such a stupid mistake but I mailed out both forms together, in an envelope with my return address. I mailed it out this afternoon, and of course I only realized my mistake after 5 PM so I can’t call anyone until Monday. Does anyone know what this means for my application? I’m so upset and nervous; I’m hoping to hear someone has heard of someone else making this dumb mistake and that it was all ok!! Thank you!

    My advice would be for your boss to do another Form 4B and have your boss send it via overnight mail on Monday morning.

    As per the instructions, the 4B needs to be mailed directly by the supervisor, so if you included it in the same envelope with your Form 1 they will probably not accept it. If another one arrives from him directly with his return address, they should accept that one and proceed along with your file.

    Always sent things to NYS with some type of delivery proof so you have backup.
    The price difference between certified mail vs overnight is about an additional 15 bucks. Well worth it in my opinion.
