I am applying for my CPA license of Illinois. I wonder whether “Certification by licensing agency/ board” form included in application form should be completed. What is this form for? If I had registered CPA of Illinois, should I completed this form?

If you have a CPA license in another state, then that state would need to fill that out for you. If this is your first CPA license, then that form wouldn't apply to you.
