I’m the only accountant in the small company I have worked for the past 15 years, however I have worked closely with an outside CPA for this entire time and she has offered to provide a letter attesting to my experience if I can provide her with a sample. Does anyone know where I can find a sample experience letter?

Check on your state licensing board's website. In Florida, its a form that has to be filled out. Likely it is different for every state.
