    Rework cost should be regarded as a cost of quality in a manufacturing company’s quality control program when they are:

    i. caused by the customer

    ii. caused by internal failure

    Correct Answer: caused by internal failure

    Shouldn’t the answer be neither. Aren’t both choices cost of failure?

    Okay, you get the “internal failure” part, somebody screwed up / damage on the production line or the process didn't yield a product that met production standards (the corn flakes aren't the right size)

    So, what's #1 really asking? It's asking if rework (costs) “caused by the customer” is a cost of quality. The answer is no, it's not a quality issue if the customer caused re-work, if's the customer asking you to go back and paint the widget blue, or cut the corn flakes in 1/2 because they want them smaller…basically, preference from the customer that added re-work cost. (and it's not a cost of quality, b/c you're going to bill them for it)
