Will you hit the adaptive phase by doing random sets of NINJA MCQ only?

You have to answer every question in the test bank right at least once, so you should click to have “never seen before”

MaLoTu – I don't think so. My understanding was you have to attempt 35% of the total questions. So you might actually get there faster by asking for only questions you haven't seen before.

Yes. You most likely will never (or very long time) hit the review phase, however, unless you create custom tests of “Never Seen Before” under “custom/sims”.
