Experience Requirements: any proof other than form 4b?

    Hello! It’s stated: “applicant must present evidence, satisfactory to the Board of full-time experience or a part time equivalent. Does anyone know if you must submit pay stubs and/or W2 as well as form 4b? I’m interested in NY state.

    I'm not sure of the NY requirements, but with Georgia, I just filled out the experience form provided by the state, attached my job description w/ duties, and had my boss sign off on those. I hope it was sufficient. My application is still in the review process. (3 weeks ago)

    You don't need to submit anything. Your employer or former employer will submit only the form 4b and attest to your experience.
